Patient Access to Records
Patient reports will be delivered to the provider portal upon completion of the requested laboratory testing. Where required by applicable law, and upon a patient’s request, results will be made available to the patient pursuant to a properly executed records request and in accordance with HIPAA, applicable federal and state laws, and any applicable laws of the patient’s country of origin governing access to medical records and test results.

Provider Account Bill, Ordering Authority, Credentialing
I acknowledge and agree that I am solely responsible for understanding and complying with all applicable federal, state, and local laws, rules, regulations, and professional standards in each jurisdiction in which I engage with patients, including through telemedicine services. This includes, without limitation, laws and regulations relating to provider account billing practices, the legal authority to order laboratory testing, scope of practice requirements, licensure obligations, and the legal authority to evaluate and treat patients within the applicable jurisdiction.
Login Holder has Permission and Authority to Make Orders and Accept Terms and Conditions
Practitioner portal accounts may be created, accessed, and managed by front office staff, office managers, administrative personnel, or other authorized representatives acting on behalf of a licensed practitioner or healthcare provider. By creating, accessing, or using a practitioner portal account, the user represents and warrants that they are duly authorized to act on behalf of the applicable practitioner or healthcare provider and to accept these Terms and Conditions on their behalf.
The user further agrees to: (i) maintain and, upon request, produce documentation evidencing that any laboratory order submitted through the portal was authorized and executed at the direction of a duly licensed and authorized healthcare provider; (ii) ensure that all use of the portal complies with applicable laws, regulations, and professional requirements; and (iii) promptly notify MDX if the user’s authority to act on behalf of the practitioner or healthcare provider is revoked, terminated, or otherwise modified in any material respect.
