An order statement containing pricing before any adjustments will be provided.

Patient Pay

Acceptable Payment Methods
Payment must accompany all specimens for processing. Test results will not be released without payment.
- All major credit cards including Visa, Master Card, Discover, American Express, JCB, and Diners Club
- Health Saving Accounts (HSA) and Flexible Spending Accounts (FSA) with Visa, Mastercard, Discover or American Express
- Check or Money Order
- Wire Transfer
Cancellation Policy
To cancel any tests, Mosaic Diagnostics must receive a written notice within 24 hours of the laboratory receiving the sample. Cancellation requests made outside of this timeframe will be subject to a cancellation fee. To request a cancellation, please email our billing department for the cancellation form.
