Records Will be Delivered to the Portal

By registering for and accessing the patient portal, you acknowledge and agree that such registration constitutes your request to receive and access your laboratory test results electronically through the portal, to the extent permitted by applicable law.

Consent to Test Submitted Samples

By submitting a sample for testing, you acknowledge and agree that you are voluntarily requesting and authorizing the performance of the laboratory test(s) ordered by your practitioner or otherwise requested by you, as permitted by applicable law. You further acknowledge and consent to the processing, analysis, and reporting of such test results in accordance with these Terms and Conditions, applicable laws, and MosaicDX policies and procedures.

Self-pay if Government Funded Insurance

If you are enrolled in or covered by Medicare, Medicare Advantage, Medicaid, Medicaid Managed Care, TRICARE, or any other federal or state government-sponsored healthcare program, you acknowledge and agree that services provided by MDX will be treated as self-pay services, and you agree not to submit, or cause to be submitted, any claim for reimbursement to such government healthcare programs.
